I've got some innocenti windows and wondering about how to fix them into the doors. The frames have 3 large rivets left in them.
Surely the rivets don't go through the outside of the door frame though? Any thoughts peeps?

Yes they do, well at least on a pair of doors I got which I think were standard doors with the Innocenti windows fitted.

It's not unusual, my long gone Dolomite Sprint had the same style openning quarterlights and that too had rivets through the frame in exactly the same places as your photo. The factory rivets were smaller than the standard 1/8" though, your windows have probably seen many drills to make the holes larger.

Was there any rubber seal in between the quarterlight and the door frame?

Just to double up on this, my Inno doors have countersunk screws in place of the rivets but yes they are going through the frame from the outside.
