I've got an engine with a 12G940 head on but a 99H engine block ID tag on it.
The thing is there are no cam follower covers as per all 998s i've seen. Can you get 998s without those?

The ID tag looks a little damaged. Is it most likely someone's swapped their engine to a 1275 but wanted it to read as a 998 on the books.

Yep seen it a few times. Most likely a 1275 with a 998 tag on it.

If something is worth doing, it's worth doing half of.

If it has two core plugs in the block above the transfer case, then it is a large bore block, most likley a 1275.
